  1. When you pay for testing, your name and email address come directly to me.
    $730 (Includes personal visit by phlebotomist to your home or office to draw blood and all of the tests listed above). Not available in California and New York where-unlike the other states-they require the physician who orders blood work to live in the state.
  2. Fill out the following form so that I can arrange a phlebotomist to call you and arrange to come to your office or home at your convenience to draw your blood:
  3. I will arrange for someone to call you within 2 business days to schedule the phlebotomist visit. 
  4. I will contact you withing 17 business days after the blood is drawn to provide you with the results.

By ordering tests here, you are agreeing that I did not promise that you would receive any re-payment from any insurance carrier or from Medicare for these tests (But, usually, insurance reimburses for testing).

If you live in another state, I'd be happy to give you an interpretation of your lab work (talk with you on the phone about your results), and write a letter to take to your local physician.  I'm not able to prescribe medications without a physical exam by me, but I can hopefully encourage your local physician to prescribe some of the therapies recommended and can explain to you what non-prescription therapies would be most helpful for you.
